Before Your Admission

When a referral is received from your doctor, surgeon or nurse, one of our experienced assessment team registered nurses will visit you at the acute hospital. These nurses meet with you, assess your ability to participate in our fast stream rehabilitation program and your ability to safely return home following our intensive rehabilitation program.

When you are medically ready for rehabilitation and a bed is confirmed, the acute hospital will arrange for you to be transferred to Donvale Rehabilitation Hospital.

You have the right to choose which rehabilitation facility you attend. If you are not sure if a referral has been made on your behalf, please do not hesitate to ask your doctor or to contact us.

If you are not in hospital, but you and your doctor believe an inpatient rehabilitation stay would benefit you, please ask your doctor to send a referral to our Day Rehabilitation office. You will need to see one of our rehabilitation physicians who will determine if an inpatient stay is warranted. Our assessment nurses cannot see you in your home.

 

Your Inpatient Stay

Rehabilitation provides the therapy, care and support needed to assist you to safely return home with maximum independence.

Our multidisciplinary team will work with you to establish an individualised therapy plan in order to achieve your goals.

A physician specialising in rehabilitation will oversee your therapy program and monitor your progress in conjunction with other team members. The nurse unit manager will coordinate your care and is available to discuss any concerns that may arise.

As you feel more comfortable during your stay, we expect that you will be able to resume your daily activities and self-care in preparation for your discharge home.

We encourage you to care for yourself as much as possible in preparation for returning home. However, we are ready to assist or supervise this process if required.

For your rehabilitation to be truly effective, it must be properly suited to you. You and your family are an integral part of the rehabilitation program and you are encouraged to ask questions, raise issues and discuss your own personal needs with staff. The rehabilitation team is committed to making your time at Donvale a positive experience for you and your family.

On a weekly basis, your treating team, led by your rehabilitation physician, will meet to discuss your

  • Rehabilitation progress
  • Plan for discharge

Your nurse will seek your input before and provide feedback to you after each meeting. There may be occasions where it would be beneficial for you to attend your case conference. If you wish to attend, please speak with your nurse.

  • Comfortable, loose fitting clothes that you can wear during therapy (e.g. t-shirt, tracksuits, shorts)
  • Supportive walking shoes
  • Reading material and glasses if required
  • Bathers if your program includes hydrotherapy
  • Toiletries and tissues
  • Nightwear, dressing gown, slippers
  • Prosthesis or gait aids if required
  • Any medications you are currently taking
  • Medicare card
  • Details of private health insurance membership
  • Pension, pharmaceutical benefits, concession, health care or Veteran Affairs' card
  • Compensation details, if your treatment is related to a workers' compensation or third party claim
  • Doctor's referral
  • X-rays if applicable

Unfortunately, we are not able to take responsibility for any valuable belongings. We strongly recommend that you leave your jewellery and other valuables at home.

Our on site kitchen offers a broad spectrum of menu choices. Our diet monitors will meet with you daily to discuss your meal options for the following day. Please inform us if you have any special dietary requirements.

Breakfast:

8:00 am to 8:20 am

Morning Tea:

10:00 am to 11:00 am

Lunch:

12:00 noon to 12:20 pm

Afternoon Tea:

2:00 pm to 3:00 pm

Dinner:

5:00 pm to 5:20 pm

Supper:

7:30 pm to 8:00 pm

Hairdresser:

We have a visiting hairdresser who can work around your therapy times.

Mail:

Your mail is delivered to your room each day. After discharge, it will be redirected to your home address.

Laundry: 

If you do not have anyone who can assist you with laundry, you may ask nursing or environmental services staff for assistance to access a laundry service.

Pastoral Care:

You are welcome to invite your own pastoral care provider to visit you in hospital. Alternatively, we can request a minister from the local community to visit you if required.

During your stay, your personal belongings will be your responsibility. We suggest you leave valuables, especially money, at home. You may choose to bring a small amount of cash in if you would like to use our hairdresser's service. If you wish to keep valuables with you, the hospital will not be held liable for any loss.

Please report any lost property to the nurse unit manager on your ward.

You will have your own telephone with a direct line for your convenience. Local calls are free. All other calls, including calls to mobile phones, will be charged to your account. Mobile phones may be used throughout the hospital.

Wifi is available for patient use. You will need to have your own device that is enabled for wireless internet.

For your safety, we are required to visually check all electrical items you may bring to hospital (e.g. hair dryers, razors, etc.). Please ask your nurse to complete a visual inspection of your electrical equipment prior to use. Any concerns will be directed to the maintenance department.

Each patient room has a television with free-to-air and Foxtel channels.

We are committed to providing the safest possible care for you during your stay with us. As such, we are vigilant in our aim to limit and prevent the spread of infection. You and your visitors have a vital role to play in reducing the risk of infection to yourselves and other patients and staff. We encourage you and your visitors to use the hand rub located throughout the hospital. Family and friends who are unwell with colds, stomach bugs or rashes should be discouraged from visiting you in hospital.

In the health interest of all patients, staff and visitors, Donvale Rehabilitation Hospital is a smoke free environment.

Unfortunately, we are unable to allow pets inside the hospital. However, you are welcome to have your pet brought onto the hospital grounds for a visit.

For your safety, CCTV cameras operate throughout the hospital.

 

 

 

When You Leave

 

Time of Discharge

Time of discharge is between 9:30 am and 10:30 am.

 

Discharge Summary

A summary of your illness/injury, treatment and care will be written by your rehabilitation team for your general practitioner. You will also be provided with a copy of your discharge summary.

 

Transport

Family or carers will be required to assist you to travel home on discharge.
